One problem was indices with 0 time before and after the signal which is not possible. There might have been something else too, but fix that and we'll see if it still fails.

Would be interesting to see stream dumps of such disks.
A generic FM/MFM controller definitely can't handle 192 bytes per sector - it can only use power of 2 sector sizes starting with 128.

Indeed, but better find out if that's the case. If it's an error like case 3 above, we'll see repeating track numbers in the log.

There is no inconsistency in the data used to generate the predictable weakbits (assuming you have a good disk/image), therefore you should see that the data matches, ie all green. Consistency: Yes, horizontal is the position on the track and vertical position is the specific revolution affected by an inconsistency.
